When a loved one’s care needs begin to change, it can feel overwhelming to know where to start. Our Guidance & Roadmap Session is designed to give you direction and peace of mind — a clear understanding of your options and what to do next.

What’s Included

Personalized Consultation (60–90 minutes) — Together, we’ll review your loved one’s current care situation, identify key challenges, and outline priorities.
Customized Roadmap Summary — You’ll receive a written overview with clear, actionable next steps in areas like housing, safety, health, finances, and communication.
Resource Guidance — We’ll discuss where to find trusted information and local supports to help you move forward, including an Assisted Living Facility Tour Guideline to help you know what to look for and what questions to ask during visits, plus a Detailed Facility Visit Questionnaire — a professional guide and comparison tool with key questions, red flags, and scoring criteria to help you confidently evaluate communities.
Follow-Up Email Summary — A written recap of our conversation and your next few action steps for clarity and focus.

This session provides a strong starting point and the reassurance of knowing you’re on the right track. If you’d like ongoing help putting these steps into action — coordinating care, vetting options, and guiding your family through the process — that’s where the Navigation & Support Package comes in.

Note: If you decide to move forward with the Navigation & Support Package after completing a Guidance & Roadmap Session, the cost of the Guidance & Roadmap Session will be applied toward your Navigation & Support Package.

What’s Included

Includes everything in the Guidance & Roadmap Session
Follow-up support provided for up to 90 days or until placement, whichever comes first.
Calls, emails, selective document review, and family meeting facilitation
Recommendations for facilities and resources that best meet your family’s specific needs
Facility Visits: I will personally visit facilities on your behalf if you don’t have the time, ensuring the options align with your loved one’s needs and preferences
Appointment Coordination: I’ll set appointments and find apartment or room availability on your behalf
Family Role Organization: Assistance with division of responsibilities for family members — bills, pills, ointments, and appointments — to create clarity and reduce overwhelm
Attendance at up to 3 key appointments (if desired)
Selective guidance on preparing and planning for your own future care — helping your children navigate these steps more easily when the time comes
Ongoing coaching and emotional support for family decision-making
